Fees

Fee-for-service is offered to clients who do not have a health insurance provider. It is a non-insurance financial arrangement. All clients must pay at the time of service. There is an option to pre-pay for packaged sessions.

Viva Physio & Wellness, Ltd will provide receipts for services paid in full. Discounted rates are made available to fee-for-service providers. Please note, this receipt will not be appropriate for, nor should be submitted to, an insurance provider for reimbursement.

Insurance

Copays, Co-insurance and deductibles must be paid at the time of service. Please call and speak with your insurance provider to understand your financial responsibility for Outpatient Physiotherapy visits.

Viva Physio & Wellness, Ltd will contact your provider to obtain verification of benefits.

You are ultimately responsible for verifying and understanding the contract between you and your health insurance provider as it relates to payments, number of visits and deductibles.

You will be responsible for the full cost of services if your provider denies coverage of your claims.

Payment

All accounts must be paid in full (inclusive of insurance payment) within 60 days of your date of service. Failure to receive payment within this timeframe will result in a 15% additional charge and handover to a collections agent.

Payment options available: cash, checks, local digital wallet, direct deposit CIBC (please send proof of payment) or payment button sent with your invoice. Returned checks will incur a $50 fee.

Cancellation Policy

Your appointment time represents 45-90 minutes of your Physiotherapist’s time reserved for you alone. Viva Physio & Wellness, Ltd therefore adheres to a strict cancellation policy.

All cancellations/reschedules, for any reason, must occur within 24hrs in advance (by 5pm on Fridays for Monday appointments) or will incur a $75 fee.

For home visits, the fee incurred is equal to the full estimated cost of the scheduled session.
